PHC Names Quality Management Director

Carol Clark, RN Carol Clark, RN Robert V. Levine, President and Chief Executive Officer of Peninsula Hospital Center, has announced the appointment of Carol Clark, RN., as the new Director of Quality Management.

Clark comes to Peninsula Hospital Center following a twenty-three year career at Jamaica Hospital Medical Center, where she most recently served as Director of Case Management, following an elevenyear career there as a medical/surgical nurse.

As Director of Quality Management at PHC, Clark will supervise the Social Work, Performance and Quality Improvement, and Case Management areas.

Under Clark's leadership, the Hospital Center has recently joined a national campaign entitled "H2H - Hospital to Home." This new program is designed to help patients with their transition home following a hospital stay, with the goal of avoiding hospital readmission when possible. "

This initiative focuses on a patient's continuity of care, something that is critical to a patient's recovery following hospitalization," explains Clark, "and we are committed to this initiative."

Clark received her Bachelor's Degree from York College and her Master's Degree in Nursing from Lehman College.

"The challenges today in any hospital setting are many, but I am very happy to be at Peninsula Hospital Center and am grateful that the hospital already has an excellent Quality Management team in place," says Clark. "The staff here at PHC goes that extra mile to make sure our patients benefit from the best possible health care."
